Phone consult redux

One year ago today, I was in a hotel room in Omaha recovering from my surgery with Dr. Hil.gers. I was 4 days post-op, in a considerable amount of pain, still undergoing the ovarian ultrasound series and all alone, missing DH who had flown back home to be with the boys. I was miserable on so many levels, but was buoyed by hope that this crazy trip was going to be worth it in the end. The ultrasound series confirmed a LUF and I eagerly looked forward to my follow-up appt with Dr. H later that afternoon where he would tell me he had found the culprit for our 10 years of infertility and a Rx for the treatment.

A year has gone by and I have found anything but peace after my experience in Omaha. Specifically, after everything that was done to me in Omaha, he declared that he could find no clear reason for our infertility (an utterly frustrating and frankly devastating fact for me to grapple with). That's not his fault, but he talked me into participating in two experimental studies: low-dose cortisol and T3 (thyroid) therapy. Both drugs, he claimed, have evidence of being "the ticket" for couples experiencing unexplained IF. After about 14 months on T3 and 7 months on cortisol, however, I asked to withdraw from both studies because of the expense involved and the weight gain I experienced. I didn't notice any improvement/difference at all with the drugs.

But what I really wasn't at peace with was the fact that I had a documented anovulatory cycle that never got addressed. I never ovulated while I was out there. His response of "well, that's probably just a fluke" has just not been sitting well with me these past 12 months. When I talked to his nurses about it afterward, they just gave me their canned response to trust him.

Sorry, but I need a better answer than that.

I didn't spend all that additional money and time away from my family last November for this. It was after this last conversation with Dr. Hil.gers' nurses in September--with whom btw I was very nice--when I withdrew from the cortisol study that he suggested a phone consult. Frankly, I was a little shocked and just sort of want to be done with all this but, sure...I scheduled it anyhow. In the mean time, I was doing a little more research of my own online, specifically regarding low-dose naltrex.one (LDN).

And so, I had my phone consultation with him two weeks ago. I've been chewing on what we talked about ever since. It was a 35-minute conversation in which I addressed three areas of concern:


1) I wanted to know generally what our "prognosis" is and where, if at all, we go from here. Has he reached the bottom of the barrel of options?


2) I wanted clarification on the results of the ultrasound series and why he didn't think LUF was a significant finding worth pursuing a little more.


3) what is the criteria for prescribing LDN? It appears that it's been used (by him and others) for the last 25 years, particularly in cases of infertility with unexplained etiology.


Re: 1) he mostly talked in generalities about what they have the ability to test for and treat. I've been tested and treated for just about everything he even remotely suspects. He was concerned about the tail-end spotting I continue to have despite the biopsies & cultures he ran last year (which came back negative) and the 21-day course of antibiotics both Mr. Mike and I took in December. He said he has a hunch that the tail-end spotting is a response to some kind of sub-clinical infection--but he doesn't know what--and that is ultimately responsible for our infertility. He encouraged me to not look at our IF journey as the 11-year journey it's been but rather just the 12 months it's been since he saw me. I get what he's saying, but he forgets that I've been a NaPro patient for 9 years.

Re: 2), he was quick to deny that he said any such thing. He reiterated that LUF is a very significant finding, but only for the cycle in which it occurs. He has no way of knowing how often it's happening but suspects it's not every cycle for me. He mentioned something about being under stress while I was out here and he's not surprised that I failed to ovulate. He talked mostly about how far advanced his ultrasound technology is and how well trained all his techs are. I was going to ask him about getting a second series done locally, but honestly--I let it drop. I know what his answer would have been. I'm going to be seeing Dr. S in a few weeks and if I get a second opinion/new series done, it's probably going to be through his office anyhow.

Re: 3) I told him I had been doing some of my own research on LDN and pointed out that Dr. Phil Bo.yle in Ireland has had remarkable success with it, especially in cases with endometriosis, tail-end spotting, PMS and other fertility related ailments. He responded that he had been Rx'ing it longer than Phil Bo.yle (for 25 years or something like that) but that he has fails to be as charmed by it as Dr. Boy.le seems to be. BUT--he was willing to Rx it for me and give it a shot. He mentioned that one thing that makes me a reasonable candidate for it is the evidence of a sub-clinical infection (tail-end spotting) and seeing as LDN boosts the immune response, it could assist the Omega-3's I'm already taking in fighting off any subtle infection that might be going undetected. It's a long shot, I know. But I adore Phil Bo.yle and if he thinks it's worth a shot for those who have unexplained IF, I'm willing to give it a try.

In all, I guess I'm satisfied by what I heard. Not surprised, not overjoyed, not completely disappointed. He answered my questions in a way I expected him to. I told Mr. Mike, "leave it to ME to be the kind of patient that stumps even Dr. Hil.gers!" I think I can safely say that he has exhausted what he can do for me though because I sensed some discomfort on the other end of the line. I'm an enigma. I don't know if I've quite reached the "finally-able-to-close-this-chapter-of-my-life" point yet--given the fact that I campaigned for the LDN and he's willing to Rx it. I'll give the LDN a chance for a few months and just continue to pray for God to guide us.

As a final interesting side note...While I was on the phone with him I asked for a Rx for Diflucan for a yeast infection I was having...probably the result of the antibiotics I was taking for that tick bite last month. I told him how highly susceptible I seem to be with getting yeast infections, especially while on antibx. That triggered an idea for him and he shuffled through some paperwork to find the culture and biopsy done on my uterus last year. He said there was evidence of yeast both there and on the Pap he did in the office the day before surgery. He is speculating that perhaps I have a chronic problem with yeast (perhaps a culprit for that sub-clinical infection he suspects??) and Rx'ed a heavier 3-day course of Diflucan in hopes of knocking it out completely.

So I'm now reading the The Yeast Connection by William Crook. And someone shoot me now please because I've also been reading Dr. Toth's book Fertile vs. Infertile for the past year and was looking up info on an office visit with him in NYC.

I'm sorry that you had been feeling so frustrated about Dr. H, but I'm glad that you have some plans on moving forward and you'll be seeing Dr. S soon. As someone commented on IATG's post, many of us are hard cases and will have to see a couple, if not many, dr. to get all our problems diagnosed and treated.

I can imagine it must be extremely frustrating for Dr. H not to have been more helpful. I am glad for you that he prescribed the LDN and though I don't know much about it, that is definitely reason for hope. I feel that Dr. H is the best first line of defense anyone could do, but it isn't the end all be all. It is the best bang for your buck and the best way to use your time initially, I feel. I think he has success with something like 86%, which is huge for IF, but it isn't 100% so please don't feel like you are the only one. I think that the yeast problem and the infection problem are two more routes to explore, so that is 3 total. That is very promising! I am glad you are reading Dr. Toth's book. I am a patient of DT and Dr. H and it took the two of them to get me to having my son (see my blog if you haven't already). Anyway, I am sorry for the long comment, but I feel that often Dr. H sends the patients he is unsuccessful to DT (when he sees a reason) and the 2 of them together have been able to treat about 75% of the folks that Dr. H hasn't been able to have full success with on his own. Again-huge wonderful number when you are considering the population! So, in summary, you have lots and lots to be hopeful about. Good luck on your journey, I will keep following and keep praying for you. I am happy to answer any questions (I did not need to go to NY for full tx-sometimes it doesn't even take that much and it saved us tons of $$$). God bless!

I could go on and on about my less than ideal experience with PPVI, but I won't. But I will just say that I can totally relate to thinking how that crazy trip would be worth it in the end.. I said that very thing so many times!

I also had a LUF cycle while doing my u/s series for them. It wasn't in Omaha, but it was in NJ, which they told me was basically the only other place in the country they'd let me do it at. Anyways, the same thing was said to me.. it might have been a fluke, it might not have been. And nothing was ever done about it. I believe I am ovulating now as far as I know and I tend to think in my case the problem was fixed by a combination of surgery and treating PCOS.

I'm sorry.. this must be so frustrating. But at least you are seeing Dr. S soon, as are the rest of us bloggers! I am so hopeful for him to figure out what's wrong with us.. I hope I am not expecting too much. But basically it's my last shot!

I am sorry that this journey is so frustrating! If you want to talk, please email me and we can get together. Also, let me know when you are coming up and if you need a place to stay!

I believe that infection played a huge role in our infertility. However, we had so many other factors that we know that wasn't the only one.

I am not a fan of LDN. I took it for a year and my opinion is that it made my infection worse. My brown bleeding didn't get any better and I think that it is better to get rid of the infection first and then work on the immune system if needed. JMHO.

I wonder how you and your DH would respond to 2 months of antibiotics? I don't know of any physician that would do this, though.

I like DT's theories but don't like how expensive it all is. The frustrating part was for us was doing everything what DT recommended and still have the darn CT! I do think that the ABX cleaned up our systems and my body wasn't fighting this infection and can heal. Who knows.

There are also less expensive ways to do DT's treatments like having an infusion company that is in your insurance's network do the IV and it can be completely covered.Just a thought. His theories do make sense for unexplained IF.

One other thing you could look into is the food intolerance testing. Dr. S may mention that to you and one of the things he wanted tested in us was for the yeast/candida.

I can't really comment on the whole PPVI situation, I have never visited there, but my NaPro Doc thinks maybe a trip will be in my future if they can't figure things out on my end.

I just wanted to chime in and say that I have been on LDN 4.5mg for the last 6 weeks and no, to my knowledge I am not pregnant yet, but it is only P+3, but I have had no side effects and it has dramatically increased my sense of well being - I am talking about sleeping so much better at night, seeming to be able to tolerate the normal stresses of life much better, etc. I take these as positives because I do think excess stress contributes to some problems with infertility in my case. I know that is not true for everyone, but I think it is definitely the case with me. Now, to be completely truthful - at the same time I was prescribed LDN, my synthroid dose was uped to 75mcg (from 50) and they also rasised by metformin to 1500 (from 1000), so my change in attitude and general well being could have been from one of them or a combination of them. Needless to say...I don't care if it was a placebo effect- but reducing stress and PMS (oh, I forgot to mention that - not one "crying" or crabby episode the week before my period this past time - crazy enough my husband even said something) - it has been worth it. I hope it goes well for you. As someone else above said...I am not sure how LDN works with infections though...as I haven't been diagnosed with anything like that - but regardless good luck! I have been right where you are....the frustration is so hard.
After three trips out to see Dr. H, three surgeries, and years of treatment, I just couldn't take it anymore. This was also after being treated by a Catholic dr in my own state who says that their practice deals with IF...though I guess I am the case that no one can figure out. I have been reading (thru blogs) more and more about Dr. S...and I am considering making a trek out to see him--much closer than Omaha! I am excited to see how your visit with him goes. Thanks for sharing your journey with us. You are in my prayers.

I just came back to read through comments since I keep forgetting to check the little box to have them emailed to me...anyway, the thing about 2 months of abx is worth checking out. We did a phone consult with Dr. Totha nd Dr. H prescribed the orals for us. Dr. Toth thought they had a good chance of working and they did. 21 days Biaxin 500 mg 2 x a day for dh and I and then CD 1-10 on cycles where we ttc the next 3 months. I did a local IV upon pregnancy just to be on the safe side, and one at delivery-both incredibly cheap. You would have to talk to them about your yeast problem, these 2 could complicate, but it may be worth a shot. Also, I had LUF while I was in Omaha and my mom was with me. She was trying to be comforting and asked if it could just be a fluke and they said in my case, no way. It was every cycle. So, it seems they don't always answer the same on that-just fyi. For my mom it was tough to hear, but for me it was comforting. Okay, this is what is wrong-do something about it! And they did. Anyway, food for thought...
